Subject: Request for issue of Duplicate Documents in respect of CSP flat No. A/ 56 DDA situated at C.R.Park, New Delhi-110019.

Dear Sir,
With reference to the above, I state that I have lost/misplaced the following original property papers of my above DDA Flats ;
(a) Demand Cum Allotment letter,
(b) Possession letter,
(c) Site Possession slip.
All 3 documents were issued by the DDA in respect of the above said flat.
I have lodged a complaint/FIR to that effect with Delhi Police ,PS Crime Branch, Delhi vide LR No. 391042/2014 dtd. 06/08/2014 (copy enclosed)
Also given Public notification in the matter on 16/09/2014 in “The INDIAN EXPRESS” & “JANSATTA”. Copies of News Paper cuttings enclosed for your kind perusal & record.
It is therefore, requested to issue duplicate copy of documents i.e. (a) Demand Cum Allotment Letter, (b) Possession letter (c) Site Possession Slip as early as possible.
