Subject : Show cause notice for insubordination & Negligence
Please be advised that it has been noticed that you have been making continuous errors while punching bills and in entering of Serial numbers of Gift Vouchers , which need to be manually entered while issuance of the same to clients .
The incidences of making errors while punching bills were raised two months back and subsequently your performance was being monitored.
A verbal warning was already issued to you by your reporting Manager (–—— ) for continuous errors that are being made while punching of bills & issuance of Gift Vouchers.
Specifically, you had made an error while punching the GVs during the Rakshabandhan promotions.
The above stated acts pertain to Negligence.
You are thereby summoned to submit your written explanation and justify as to why disciplinary action should not be taken against you.
Your explanation should reach the undersigned within 48 hours of receipt of this letter.
