Subject: Show cause notice for Insubordination
It has been reported against you that on Date-Month-year when you were attending to a client your
behavior and attitude towards attending the client was not positive and you were not responding to the
queries of the client in an appropriate manner.
For the above stated incident, the _______ Manager (Designation) – ____________(NAME) – your
Reporting Manager, also tried to verbally counsel you that your behavior towards clients should improve
but instead of understanding the counselling and taking the same as a feedback to improve you started
arguing with him.
The above stated incident was not the first incident wherein your misbehavior was observed. Prior to this
it was reported that you had proceeded for unapproved leaves without providing proper explanation for
the same.
The above stated acts pertain to insubordination & misconduct.
You are thereby summoned to submit your written explanation and justify as to why disciplinary action
should not be taken against you.
Your explanation should reach the undersigned within 48 hours of receipt of this letter.
