It is brought to your kind notice that our establishment had been brought under the purview of the EPF & MP Act, 1952 with effect from January _________ under Section l(4) and since then we have been complying regularly.
As per your instructions, we are required to file Form No.5A on line but while submitting Form No. 5A, the mandatory fields shown against “Business Activities and ownership details” do not match with our activities and hence suitable amendments are required in your mandatory field. However, you are requested to kindly go through our voluntary coverage application and suggest the necessary particulars to be filled against the mandatory field “Business Activities and Ownership details” of Form No.5A for immediate compliance at our end.
