Subject: Show cause notice for Negligence
This notice is in strict reference to the incidences reported against you. Please find below the narration of events that amount to Negligence in attitude and breach of company policy for leave approvals.
Negligence in attitude: Taking leaves without approval – As per the Company Policy, you are supposed to get your leaves approved by your immediate reporting Manager, and the same needs to be approved by the HOD on Leave Portal. Your leave for 15-Nov-2017 was not approved inspite of that it was reported that you absconded from work on the said date i.e. 15-Nov-2017. It has also been observed that the leave request was submitted by you very late and that was also submitted by you upon enquiry by your reporting manager on 19-Nov-2017. The leaves were requested for grand parent’s funeral however the leaves were extended when the leaves were supposed to be asked for re-consideration.
Keeping in view the urgency of projects, the extension of leaves for your cousin’s wedding is not approved by the Reporting Manager.
Absenting yourself from duties without prior intimation is a misconduct for which you are making yourself liable for necessary action.
This is a written warning letter issued to you for your negligence and misconduct. You need to resume your duties immediately or within 48 hours of receipt of the letter at KAMA designated Store.
