Subject: Show cause notice for Negligence/ Insubordination & Misconduct.
This notice is in strict reference to the incidences reported against you. Please find below the narration of events that amount to Negligence in attitude, Insubordination & Misconduct.
1. Negligence in Attitude: Reporting late at work shifts: On 1st of January:- during the staff meeting it was made clear that as per the Company SOP for reporting to work in time shall have to be followed. Inspite of that it was observed that you were not punctual in reporting to work. The dates on which you reported late without substantial reasons were 10th Jan. , 14th Jan. & 16th Jan. Also every alternate day it is observed that you report late on duty.
2. Insubordination : On 20th Jan: you were instructed to report to duty for arranging a refresher training of staff at the hall but you did not report to work and were absent without any information. Upon being called by your reporting Manager you stated you shall be unable to come. As per the company policy if you are unwell or can’t report to work on duty then the Reporting Manager should be kept informed of the same prior to the reporting time and not when the Reporting Manager follows up for your absence.
3. Misconduct: One 14th Jan:-it was reported that your behaviour was inappropriate towards one of the staff and was impolite while reverting to him / her towards formulation of a query of one of the client. There have been incidences reported towards your inappropriate and impolite behaviour towards another staff on 15th Jan.
You were verbally informed for improving your behaviour. However the same did not improve.
You are thereby requested to submit your written explanation and justify as to why disciplinary action should not be taken against you.
Your explanation should reach the undersigned within 48 hours of receipt of this letter.
